The Certified Home Health Agency (CHHA), based in our Nursing department, provides full-scope nursing care in the home setting, and fulfills NYS Department of Health’s Division of Home and Community-based Services regulations. Patients are referred by physicians from local hospitals and clinics.
The LSAFHS nurse assesses, plans, and delivers hands-on clinical care to a population of first-time mothers, high-risk maternity patients, infants, pediatric patients, and the elderly. Our visiting nurses incorporate the home and family and what home and family mean into the patient’s plan of care, whether skilled nursing is needed to manage an illness or the transition through pregnancy into parenthood.
Our nurses practice holistic, patient-centered care and are advocates for those we serve. Knowing that health restoration doesn’t begin or end only in the hospital and that the patient’s needs might be broader than what skilled nursing delivers, we refer the patient to other LSAFHS programs – the food pantry; breastfeeding and parenting workshops; Parenting and Child Development program; environmental services; advocacy services; and, to community resources outside our agency.
Our partners rely on LSAFHS’s nurses to deliver care to their patients with skill, compassion, and warmth.
